I apply for a trial and potentially membership to practice Bushin. I shall never use the name “Bushin” or teach its techniques to any non-members without your authorisation even after I withdraw from membership.The practice and training of the discipline of Bushin involves actions which may result in intentional or accidental physical contact or other situations involving unintended personal injury or death without negligence on the part of the person causing the injury and I understand that I participate in the discipline of Bushin entirely at my own risk and absolve the organisation and my fellow members from all liability in respect thereof. I declare that I am in good health; I am not suffering from any medical condition and am not aware of any circumstances that make my participation in Bushin unsafe and agree to notify Bushin if I become aware of any such condition or circumstances in the future.
